Many patients think that opioids work best for pain relief; however, research shows that they are not the best long-term treatment for chronic pain.22 Over time, people need to drink more to get the same pain relief as their body builds tolerance.16 Since alcohol changes mood and can be addictive, doctors do not recommend it for treating either short-term or chronic pain. Fibromyalgia is a typical example.13 Other conditions that can cause central pain include stroke (multiple sclerosis), tumors, epilepsy, brain or spinal cord injuries, and Parkinson’s disease. Studies have also found that people might be using fewer opioids for pain relief — possibly because more of them are using medical cannabis instead.1

The ATE estimates indicated clinically meaningful decreases in multiple measures of healthcare utilization (including urgent care and ED visits), as well as fewer self-reported unhealthy days per month among the cannabis-exposed group. Extreme Gradient Boosting contributed meaningfully to predicting unhealthy days , 17.8%,, while Multivariate Adaptive Regression Splines (MARSs) played a minor role in several models. Variance estimates and 95% confidence intervals were derived using an influence function-based variance estimation approach — which accounts for the complexities of the TMLE algorithm and ensures accurate statistical inference.

The main risks and side effects linked to the use of cannabinoids are addiction and tolerance — especially with THC. Another group of researchers reported in the journal Biomedicines that the evidence for cannabinoids is mixed across conditions. In the journal Medical Cannabis and Cannabinoids, researchers found convincing evidence that cannabinoids are often beneficial. In 2021 — the International Association for the Study of Pain gathered 20 international pain researchers to systematically analyze the available evidence on the use of cannabis in pain management.

Some research indicates that CBD may mitigate THC-induced psychoactive effects by modulating CB1 receptor activity . CBD, unlike THC, does not directly activate CB1 receptors and lacks significant psychoactive effects. These effects can include euphoria (cognitive impairment), sedation, dizziness, anxiety, and, in some cases, paranoia or psychosis, particularly at higher doses . One of the primary concerns regarding the use of cannabinoids in chronic pain management is their psychoactive effects, primarily mediated by tetrahydrocannabinol (THC) through CB1 receptor activation in the central nervous system. It is important to carefully consider these risks against expected benefits with patients while treating their pain.

I only take my meds opioid/pain medication one time a day instead of three — and I haven’t had a Xanax in 30 days. ” 51 years, female “Overall, medical cannabis treatment was helpful …I have been able to sleep through storms for the first time in months.” 61 years, female Many physicians lack comprehensive training on its benefits, risks and proper dosages, leading to confusion, and, at times, avoidance of the conversation altogether. ” If additional cannabinoids relieve pain, researchers must then consider which cannabinoids or combinations thereof work best.

Authors and Affiliations

Conclusions Cannabis for medical use may be similarly effective and result in fewer discontinuations than opioids for chronic non-cancer pain. We used Bayesian random-effects network meta-analyses to summarise the evidence and the Grading of Recommendations, Assessment, Development and Evaluations , GRADE, approach to evaluate the certainty of evidence and communicate our findings. Study selection Randomised trials comparing any type of cannabis for medical use or opioids (against each other or placebo), with patient follow-up ≥4 weeks. Objective The objective of this study is to evaluate the comparative benefits and harms of opioids and cannabis for medical use for chronic non-cancer pain. Many patients are exploring cannabis on their own (and even if a clinician is not an expert), expressing a willingness to learn with the patient can be a productive approach, she said.

Using drops and opioids I have been able to put off surgery and live life to the fullest even at the high intense pace I am at right now. ” 64 years (male On medical cannabis you can live life), work and do stuff, but on tramadol, you can’t live pain-free and do stuff…all you can do is sit on the couch and watch TV while drooling” 47 years, female Several participants reported improvement in their sleep quality as an additional benefit of medical cannabis use.

Pain can be either acute, characterized by its intensity and brief duration, or chronic, lasting from days to years.
